OCS STAFF: If none of these appointment times work for the participant then--

1.Please call the assigned W4K's EAP Counselor as listed below.

2. If that counselor is not available when you call, then please

a) Leave the EAP counselor a detailed voice mail indicating that this is an OCS referral and include the participant's name, phone number, address and email and b) Please inform the participant of the EAP Counselor's name and phone number so the participant knows who their W4K's Counselor is and they have the contact information.
